SUPPORT LAPD SWEEPSTAKES - OFFICIAL RULES
The SUPPORTLAPD SWEEPSTAKES ("Sweepstakes") shall consist of twelve (12) promotions (each, an "Event, and collectively referred to herein as the "Events"). Each Event consists of the selection of "Event Contestants" (as defined below) to participate in an on-site at SUPPORTLAPD. The Sweepstakes "Entry Period" starts at 12:00:01 am Pacific Time ("PT") on 5/25/05 and closes at 8:00:00 pm (PT) on 6/25/05. The sponsors of this Sweepstakes are MY YELLOW PAGES PLUS, 543 Ave 26, Los Angeles, CA 90065. The Sweepstakes shall be governed by these Official Rules, and by entering, all participants agree to be bound by the terms and conditions herein.
ELIGIBILITY: THE SWEEPSTAKES IS OPEN ONLY TO LEGAL U.S. RESIDENTS RESIDING WITHIN SOUTHERN CALIFORNIA (SPECIFICALLY DEFINED AS THE FOLLOWING CALIFORNIA COUNTIES: LOS ANGELES, ORANGE, VENTURA, SAN BERNARDINO, AND RIVERSIDE COUNTIES), EIGHTEEN (18) YEARS OF AGE OR OLDER AS OF ENTRY DATE WHO POSSESS A VALID CALIFORNIA MOTOR VEHICLE OPERATOR'S LICENSE IN GOOD STANDING AT THE TIME OF ENTRY AND PRIZE AWARDING (AS DETERMINED BY SPONSOR IN ITS SOLE DISCRETION). Employees of the Sponsor, SUPPORTLAPD and MY YELLOW PAGES PLUS, their respective parents, affiliates, subsidiaries, related companies and advertising and promotions agencies, and their immediate family members (defined as parents, children, siblings and spouse and their respective spouses) and those living in the household of same, whether or not related, are not eligible to enter or win a prize.
This Sweepstakes is void outside the specific California counties specified above and where prohibited by law. All applicable U.S., state, and local rules and regulations apply.
HOW TO ENTER: Beginning at 12:00:01 am PT on 5/25/05, visit www.losangelespolicedepartmentsweepstakes.com (the "Entry Site") and follow the directions to complete and submit the online entry form to receive one (1) entry. TO BE ELIGIBLE, COMPLETED ENTRY FORMS MUST BE DEPOSITED BETWEEN 5:40:00 PM (PT) AND 8:00:00 PM (PT) ON THE DAY OF THE APPLICABLE TUESDAY NIGHT GAME.
LIMIT ONE (1) ENTRY PER PERSON OR EMAIL ADDRESS PER ENTRY METHOD PER EVENT DRAWING, FOR A MAXIMUM OF THREE (3) ENTRIES PER EVENT DRAWING. . Illegible, incomplete, inaccurate, or duplicate entries of a single entry method and mechanically reproduced entries will be disqualified. All entries become the property of the Sponsor and will not be acknowledged or returned.
LIMIT ONE (1) ENTRY PER PERSON OR EMAIL ADDRESS PER ENTRY METHOD PER EVENT DRAWING, FOR A MAXIMUM OF THREE (3) ENTRIES PER EVENT DRAWING. . Illegible, incomplete, inaccurate, or duplicate entries of a single entry method and mechanically reproduced entries will be disqualified. All entries become the property of the Sponsor and will not be acknowledged or returned.
EVENT CONTESTANT SELECTION: One (1) potential Event Contestant will be randomly selected from among all eligible entries received via the Online Entry Method for the applicable Online Entry Period. The Potential Online Event Contestant will be notified by telephone by Administrator on the day of the drawing and must be reachable by telephone (within two attempts) at the time of the notification call or his/her place as an Event Contestant will be forfeited and, at Sponsor's discretion, an alternate Online Event Contestant shall be selected.
All Event Contestants are subject to verification of eligibility and compliance with the terms of these Official Rules. All Event Contestants will be required to complete and sign an Affidavit of Eligibility/Release of Liability and, except where prohibited by law, a Publicity Release form within the time period designated by Sponsor or its designee prior to the Event or the Event Contestant will be disqualified and at Sponsor's sole discretion, an alternate Event Contestant will be selected.
Eligible non-winning entries from one Entry Period drawing will not "roll-over" into subsequent Entry Period drawings. All drawings shall be conducted by and/or overseen by West Coast Marketing, an independent judging organization, whose decisions are and shall be final and binding in all matters related to this Sweepstakes. Only one (1) prize will be awarded to any one individual, household, or e-mail account during this Sweepstakes. The odds of being selected as an Event Contestant for a specific Event will be determined by the total number of eligible entries received for the applicable random drawing.
EVENT PRIZE DELIVERY: Each Event Winner (and Second Chance Sweepstakes Winner, if any) shall be required to pick up his/her prize at a location designated by the Sponsor.
Prizes are not transferable, assignable, or redeemable for cash. No substitution for a prize by the winner will be allowed. Sponsor reserves the right to substitute a prize of comparable or greater value, at its sole discretion. All federal, state and local tax liabilities are the sole responsibility of the prize recipients.
NOTE: Internet entry must be made by the entrant only at the Entry Site. Entries made by any other individual or any entity, and/or originating at any other web site or e-mail address, including, but not limited to, commercial sweepstakes subscription notification and/or entering service sites, will be declared invalid and disqualified for this Sweepstakes. The use of any device or computer software to automate the entry process is prohibited.
PRIVACY INFORMATION: Personal information obtained from offline entrants will be used for Sweepstakes purposes only. Online entrants to the Sweepstakes will be given the opportunity to "opt-in" to receive commercial e-mails from dodgers.com. Eligibility to participate in the Sweepstakes is not dependent upon entrant's consent to receive such e-mails.
